Job Description Job Description Minimum 1 years nurse practitioner experience required. There is an open Per Diem Nurse Practitioner (NP) Home Health Flexible Schedule job in Windham, CT. The Per Diem Nurse Practitioner (NP) will conduct home health risk assessments within Windham County, CT. The average home health risk assessment will take about 45 minutes and the nature of the visit may vary and could include an annual health assessment or post discharge visit. The home health risk assessment will be conducted on Medicare Advantage plan members residing in Windham, CTand the surrounding Windham County, CT.Windham County, CTincludes locations like Killingly, Plainfield, Putnam and Windham. Candidates could be scheduled in any one of these locations during an individual shift.
