Job Description & Requirements Specialty: Geriatrics Discipline: Nurse Practitioner Start Date: 05/16/2025 Duration: 52 weeks 40 hours per week Shift: 8 hours, days, evenings, flexible Employment Type: Local Contract Full Time - Nurse Practitioner Albany, NY Pay - $60k to $ 120k / Annually We’re seeking an experienced Nurse Practitioner to deliver primary care services to residents in assisted living communities—improving outcomes and enhancing quality of life. Key Responsibilities: Perform comprehensive health assessments and manage care plans Diagnose and treat chronic and acute conditions Prescribe medications and order diagnostic tests Educate patients and families on wellness and disease prevention Maintain accurate clinical documentation (eClinicalWorks experience a plus) Requirements: Master’s in Nursing and completion of an accredited NP program Active New York NP license and DEA certification Medicare/Medicaid provider eligibility 3 years of clinical experience (geriatric or long-term care preferred) About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
