Job Description Job Description Minimum 1 years nurse practitioner experience required. There is an open Part Time Nurse Practitioner (NP) Home Health Flexible Schedule job in Independence, IA. The Part Time Nurse Practitioner (NP) will conduct home health risk assessments within Buchanan County, IA. The average home health risk assessment will take about 45 minutes and the nature of the visit may vary and could include an annual health assessment or post discharge visit. The home health risk assessment will be conducted on Medicare Advantage plan members residing in Independence, IAand the surrounding Buchanan County, IA.Buchanan County, IA includes locations like Quasqueton and Fairbank.Candidates could be scheduled in any one of these locations during an individual shift.
