Job Description

We are seeking a skilled Family Practice Nurse Practitioner for a 60-day locum tenens assignment at a Multi-Specialty Clinic in Maryland, starting 06/02/2025. The ideal candidate will work Monday through Friday, from 8 AM to 5 PM. This role involves providing comprehensive primary care services in a long-term nursing home facility. Responsibilities include acute and chronic care management, treatment planning, and monitoring for early changes in conditions. The NP will handle scheduled follow-up visits, monthly checkups, acute care, admissions, and discharges. Experience with dementia and psychiatric cases is essential. The provider is expected to achieve 25 RVUs and will use GEHRIMED and PointClickCare systems. This opportunity offers a chance to work in a busy, high-acuity setting with potential for extension. Job Details: Location: Maryland Facility Type: Multi-Specialty Clinic (Long-term nursing home) Start Date: 06/02/2025 LOA: 60 days Provider Type Needed: Nurse Practitioner (Family Practice) Schedule: Monday - Friday, 8 AM - 5 PM Responsibilities and Duties: Provide comprehensive primary care services for long-term nursing home residents Manage acute and chronic conditions for a high-acuity patient population Develop and implement treatment plans for residents Monitor and identify early changes in patient conditions Conduct scheduled follow-up visits and monthly checkups Handle acute care cases as they arise Perform patient admissions and discharges Utilize GEHRIMED and PointClickCare systems for patient management Collaborate with interdisciplinary team members to ensure comprehensive care Provide care for patients with dementia and psychiatric conditions Achieve daily target of 25 RVUs (relative value units) Stay updated on geriatric care practices and treatment modalities Additional Information: Active Maryland NP license required Experience in long-term care or geriatric settings preferred Familiarity with GEHRIMED and PointClickCare systems a plus Strong clinical skills in family practice and geriatric care Excellent communication skills for patient and team interactions Ability to work efficiently in a high-turnover, high-acuity environment Commitment to providing high-quality, patient-centered care for elderly residents Apply now to learn more!
