This role offers the opportunity to provide vital care to patients in the comfort of their homes, working both independently and collaboratively with a supportive team. Key Responsibilities: Perform home visits and clinical assessments Complete OASIS documentation thoroughly and accurately Utilize Homecare Homebase for charting and updates Coordinate with the care team to develop and follow care plans Educate patients and families on disease management and treatment Participate in rotating on-call responsibilities Qualifications: Active RN license in Pennsylvania Minimum 2 years of recent home health experience Proficient in OASIS and Homecare Homebase BLS certification (AHA) Reliable vehicle, valid drivers license, and current auto insurance Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission Why Butler, PA: Located north of Pittsburgh, Butler blends small-town charm with easy access to city amenities.
