Job Description & Requirements Specialty: Geriatrics Discipline: Nurse Practitioner Start Date: 06/26/2025 Duration: 52 weeks 37 hours per week Shift: 8 hours, days Employment Type: Local Contract About the Role Are you passionate about serving the geriatric population? We are seeking a compassionate and skilled Nurse Practitioner (NP) to deliver high-quality, patient-centered primary care services to residents in assisted living and long-term care facilities. This is a meaningful opportunity for providers who are dedicated to improving the health and quality of life for older adults in a supportive, team-based environment. Key Responsibilities Conduct comprehensive health assessments including physical exams, medication reconciliation, and review of medical histories. Develop and implement individualized care plans in collaboration with interdisciplinary teams. Manage chronic and acute health conditions with timely interventions and appropriate referrals. Order and interpret diagnostic tests (labs, imaging, etc.) to guide clinical decisions. Prescribe medications and treatments in accordance with state scope of practice and facility policy. Initiate and participate in chronic care management programs. Educate residents and families on health maintenance, disease prevention, and self-care. Participate in quality improvement efforts and interdisciplinary care meetings. Maintain clear, accurate documentation in the electronic medical record (EMR) system. Qualifications Master’s Degree in Nursing from an accredited Nurse Practitioner program. Active and unrestricted New York State NP license . Valid DEA registration for prescribing.
