6 Months Contract Executive (Population Health Management) HZT The executive will support the Population Health Management (PHM) leadership in developing and maintaining comprehensive population health profiles. Their contributions will be essential to improve care quality, outcomes, equity, and optimise system value for defined patient populations and company's residents. The post holder will support the team in the following areas: Drive population profiling to inform system planning and care delivery to become more systematic and targeted, and Effective use of population health data to draw insights for strategic planning Key areas of responsibility: Assist in the development and maintenance of PHM profiling products Assist in the consolidation and reporting of Population Health indicators performance Strategic planning and facilitation for Population Health Data and/or Analytics User Group and Health Assist in data and/or data analytics requests for PHM Job Requirements EDUCATION Bachelor in Mathematics, Public Health or Statistics would be preferred. EXPERIENCE Experience in population health or healthcare planning roles preferred Working knowledge of Microsoft Office is required and experience in data analytics and visualization tools will be an advantage. OTHERS Strong written, analytical and communication skills; Good people management skills and ability to manage multiple projects with effective time management. Ability to work well independently and as a team player.
