Job Description Job Description Dynamic is seeking professional, reliable, and caring Home Health Vocational Nurse (LVN) to join our Medicare-certified Home Health Care Team! We have an Immediate opening for a PER-DIEM LVN Home Health Nurse to provide Home Health visits to our patients across the Greater Los Angeles and Ventura Areas. Responsibilities: Implement plan of care initiated by the Registered Nurse Provide accurate and timely documentation, consistent with the plan of care Assess and provide patient and family/caregiver education, and information pertinent to diagnosis and plan of care Use equipment and supplies effectively and efficiently Be responsible for following all company policies and procedures Perform other duties as assigned Qualifications: Previous experience in Home Health Nursing Care - One (1) Year Ability to build rapport with patients Strong problem solving and critical thinking skills Ability to thrive in a fast-paced environment Bilingual in Spanish a huge plus!
