In this role, you will: perform comprehensive patient assessments create and implement personalised care plans deliver evidence-based treatments to optimise patient outcomes collaboration with multidisciplinary teams to provide holistic care maintain accurate patient records Requirements & Qualifications: To be successful in this role, you will need: a current and valid NMC or SWE registration and pin experience working with adults with complex mental health needs strong communication and interpersonal skills the ability to work independently and within a multidisciplinary team a commitment to delivering high-quality patient care a full UK driving license and access to a vehicle Working Hours & Contract Duration: This role offers a flexible working position requiring availability from Monday to Friday , with flexibility where possible.
