Job Description Job Description Looking for an experienced Physician Assistant/Family Nurse Practitioner at a FQHC facility in Arleta/North Hills. Providers are integral members of the clinical team. The Physician Assistant/Family Nurse Practitioner provides cost-effective services to low-income families and individuals, with a special focus on delivering culturally and linguistically appropriate health and wellness services to the residents of the community. The Physician Assistant/Family Nurse Practitioner must be committed to delivering timely, efficient, quality medical care to all patients in a professional, caring, and ethical manner. Responsibilities: Performs history and physical examinations on adults and children. Physical exams include pre-employment, school, sports, well child periodic screening, etc. Counsels for birth control and prescribes as appropriate. Gathers complete medical history and performs physical exams on each client. Provides treatment for and documents various related diseases. Writes prescriptions as deemed appropriate. Reviews results of lab tests and diagnostic imaging and arranges appropriate follow-up. Provides health education and appropriately documents all health education provided. Reviews referral/consultant reports and arranges appropriate follow-up. Participates in provider meetings and clinic-wide discussions. Participates in chart review and quality assurance activities as detailed in the Quality Assurance policies and procedures. Responds to calls from pharmacies regarding prescriptions and refills, physicians, and patients requiring medical advice. Assists the front office staff in making appropriate arrangements for patients who arrive when the provider they were scheduled with has been canceled, including reviewing the medical record and determining how soon and where the patient should be seen. Attends Practice Management and Quality Control Meetings. Required Qualifications: Master’s degree from a four-year college and/or a professional certification beyond a four-year college Certificate of completion from an accredited Nurse Practitioner Program Unrestricted California State Medical License Current California DEA certificate First Aid / CPR Certificate. Bilingual in English/Spanish EMR Proficient Medical Specialty: Geriatrics Pediatrics Primary Care Urgent Care Physical Setting: Clinic Outpatient Telehealth Urgent care center Schedule: 8-10 hour shift Day shift Monday to Saturday Weekend availability Location: Arleta/North Hills
