Job Title: Managed Care Community Connector Location: Remote Field Visits Duration: 6 Months (possible to extend or TEMP TO HIRE) Shift: Monday to Friday 8:30 am to 5:00 pm EST Pay Rate: $27- $27.91/hr. on W2 HIRING 2 TEMPS IN THE NYC – 1 TEMP IN SYRACUSE AND 1 IN NYC AREA FOR WESTCHESTER AND LONG ISLAND. Job Description: Must Have Skills: · Strong verbal and written communication skills, with the ability to engage compassionately and clearly with members. · Active listening and empathy to build trust with vulnerable and underserved populations. · Organizational and time management skills to schedule, conduct and document multiple home visits. · Ability to work independently in the field while collaborating effectively with Care Management team. · Cultural Competency and sensitivity when working with diverse backgrounds and community needs. · Basic proficiency with mobile devices and Microsoft platforms. · Bilingual is preferred (Preferred languages: Bengali, Russian, Chinese, or Spanish)
